1. login to www.google.com/adsense then click "sign up now".
2. Fill out all the form.
2.1 Payee name is the most important information to fill out.
2.3 then after fill out click the "Submit information".
3. wait for the notification email on your email then confirm it. and this is a preview look like on your account upon completing your account.
